Lose your original Waiver? Make a mistake and need a new copy? Whatever the need, CMH Heli-Skiing offers below 2 options, making it easy to complete your Heli-Skiing Application and Waiver.

Remember: CMH requires you to mail the original copy of the Waiver with all signatures. Faxes, Scans and Emails are not acceptable.

Option 1 - Digital Waiver - English Only

• Open the Digital Waiver & complete all highlighted areas BEFORE printing.
• Before printing, set Page Scaling to "Shrink to Printable Area".
• Print the form. (Note - you cannot save the completed form)
• Both applicant and witness must sign where indicated.
• Mail the completed original form to CMH (see address at right).
  REMINDER: We cannot accept scanned, faxed or email copies.

Option 2- Print Waiver - English, French, German, Italian, Spanish

• Open and Print the waiver in the language of your choice.
• Before printing, set Page Scaling to "Shrink to Printable Area".
• Complete the form in pen.
• Both applicant and witness must sign as indicated.
• Initial each page of the document.
• Mail the completed form to CMH (see address at right).
